What We're Looking For

• Links to CMS-0057-F prior authorization metrics pages or PDFs
• Any health plan required to report: Medicare Advantage, Medicaid MCO, CHIP, QHP, or state Medicaid FFS
• Data covering calendar year 2025 (the first reporting period)

We verify every submission before adding it to the dataset. All data in the tracker links back to the original source document.
